Subscription Policy:

• Subscriptions received prior to November 30 will be for the following calendar year.
• Subscriptions received after November 30 will be for 6 issues, starting after payment is received.
• Customers desiring calendar year subscriptions after November 30, will need to request and pay a $15.00 late fee per issue for back issues.

Operative Dentistry on CD-ROM:

• Archive of Volumes 1-29 available on 3 CD-ROM’s for $275.00 per set.
• Archive of Volume 30 or 31 on CD-ROM for $50.00.

Claims Policy:

• Claims for missing issues accepted within 6 months of issue date.
• Due to the availability of Operative Dentistry on-line, there will be a $15.00 processing fee for paper issue claims, payable with the claim.
• Duplicate copies will not be sent to replace ones undelivered due to failure to notify the publisher of a change of address. These requests will be handled as Back Orders.
• Claims policy note: We are a not-for-profit organization and the only paid personnel we have are three part-time office staff. The subscription fees we are charging represent the lowest rates we feel can be charged and still offer a first-class product. All other fees (like the $15.00 processing fee) are charged to cover the minimum cost to Operative Dentistry to provide the additional services. These additional fees must be paid before the additional services can be rendered. If we feel that an error on the part of Operative Dentistry has occurred, we will cheerfully refund the payment.

Back Issues:

• Back Issues from 2000 forward (some issues will be printed based on availability) for $35.00 per issue (air-postage included).

Late Fee:

• A late fee of $15.00 PER ISSUE will be assessed for subscribers desiring to backdate their subscriptions to Issue 1.

Method of Payment:

• Visa, MasterCard, American Express and Discover cards - payments should be made using the secure online payment process. Credit card payments may also be faxed to:

• Wire Transfer - Operative Dentistry charges $25.00, in addition to the subscription price, for processing wire transfers. Please contact the editor for more information on submitting payment using wire transfers. Please note that the Subscriber is responsible for all wire charges from their bank. When using the Wire Transfer option, you must email or fax the date of the transfer and the name of the bank processing the transfer to Operative Dentistry.

Online Access Notes:

• The following information is required for institutional subscriptions. Failure to provide this information may result in our being unable to access your subscription.
  • Institution address
  • librarian/system administrator name and title
  • telephone and fax number
  • email address for administrator
  • complete IP addresses
• For individual subscriptions we require the individual’s full mailing address and email address.

Online Access Help:

• All IP addresses must be accessible to the Internet; in other words, they must not be behind a firewall or proxy server.
• If your client is operating a firewall or proxy server please only submit its IP address(es).
• The computer that is used to access the Journal must accept cookies. If “cookies” are not allowed your client will not be able to access Operative Dentistry full text. they may use the following settings to allow their firewall or proxy server to accept Journal cookies.

Privacy Policy:

• Operative Dentistry does not sell or lease its mailing list to any other entity for any reason. Personal financial information sent to our offices (such as credit card numbers, etc.) is destroyed upon completion of the transaction and/or regulatory processes.

Frequently Asked Questions:

1. What are the limits of an authorized individual online user?
   • There are no limits to access for individual users for their subscription period.
2. What are the limits of an authorized institutional user?
   • Institution access is limited only to the IP addresses that they have submitted.
3. When a customer subscribes to the online journal, how far back can they go to access archived issues?
   • Perpetual access is granted for the paid subscription period.
4. When a subscriber does not renew the subscription, can they access the subscription volume in the future?
   • Yes, subscribers have perpetual access to the paid subscription period(s).
5. Is there a usage statistics service for institutions?
   • At this time we do not have Institutional Usage Services.
6. Is there a site license? If so what is the web address? Also, what is the limit of the site location?
   • There is not a license that has to be signed. The limit for access is by geographic location; for example, at an educational institution, access is only for the single campus. There is pricing for additional campuses.
7. Is there a multi site license plan? If yes, what information do you need?
   • There is pricing available for multiple campuses. There is also a new option for a virtual network. For the virtual network we require proxy server access, there is a limit of 10 campuses, and all students and staff must be required to log in by secure access.
8. Is an authorized user able to access from remote location? Are users allowed remote access?
   • Individual subscribers can log on from any location with their customer ID number. Remote access is allowed only for virtual network subscribers through proxy server.
9. Do you allow or offer Interlibrary Loan for the online journal?
   • No, we do not. We do obviously allow interlibrary loan of the paper journal.
